We welcome you to join our Learning Collaborative Coffee Break in February. We’ve had tremendous success bringing our Learning Collaborative participants together on a monthly basis during our Coffee Break sessions. This month, we will focus on your best practices for supporting discharge planning for your patients and ensuring nutrition is incorporated and even prioritized. Participants are encouraged to attend and bring their questions, suggest ideas to other participants who are encountering obstacles, or listen in for inspiration.
